Who provides your coaching
Chronic Wellness is a trading name of Sarraf Healthcare Services Ltd, company number 17421061. Our business address is 70 Salmon Street, London, NW9 8PU. Contact seekchronicwellness@gmail.com for bookings, cancellations, complaints or questions. In-person appointments are arranged separately; please do not attend this address without confirmation.
Sessions are personally delivered by Hadi Ali, a GPhC-registered pharmacist and certified longevity coach. The company is not VAT-registered; VAT is not added to the prices shown.
Who the service is for
Coaching is for adults aged 18 or over. If you will attend from outside the UK, contact us before booking so we can confirm whether we can offer the service in your location. Availability is subject to suitability, professional scope and applicable requirements.
What the service includes
We provide personalised lifestyle and longevity coaching focused on nutrition, movement, sleep, stress, habits and practical behaviour change. We agree priorities and realistic actions with you. Outcomes vary and are not guaranteed.
The free 10-minute discovery call is a practical conversation about suitability and how the service works. It is not a coaching consultation and does not include personalised health recommendations.
Coaching does not diagnose conditions, prescribe, change or stop medicines, provide emergency care, or replace your GP or specialist. Continue following your prescriber’s advice. Where medical review is needed, we will direct you to an appropriate healthcare professional.
Appointments and payment
The Free Discovery Call is 10 minutes and £0. A Personalised Wellness Session is 60 minutes and £70. A Progress Check-In is 30 minutes and £40, for returning clients who need focused additional support. There is no compulsory follow-up programme.
Paid appointments are payable in full when booking through Square. Check the selected service, date, time zone and price before confirming. Keep your booking confirmation and the terms supplied with it.
Recharge & Refocus is three prepaid 60-minute Personalised Wellness Sessions for £200, saving £10. It is the same service, not a separate programme. Pay £200 upfront through the Square payment link on our website. Payment does not book an appointment: after paying, email us to arrange each session at a mutually available time. You do not need to book all three at once, and unused sessions have no expiry. We track the sessions used and remaining.
Preparing for your appointment
After booking a coaching session, we manually email your consent documents and intake instructions. Please complete consent and return the relevant intake information before your appointment using the protection instructions supplied. Contact us if you need help or would prefer to discuss sensitive information privately.
Online sessions use Google Meet. Join from a private, safe place, not while driving. We do not record sessions. If the connection fails, email us so we can agree how to continue or rearrange any undelivered time.
Give accurate information relevant to safe coaching and tell us about changes. We may pause or decline coaching where it is unsuitable or outside our scope. We will explain the reason where possible and address payment for undelivered services fairly, in line with your rights.
Support between appointments
Routine, non-urgent email support is available while you have a future coaching session booked. This provides extra lifestyle guidance between appointments, not a replacement for a full review. Hadi replies when available; messages are not continuously monitored and immediate replies are not promised.
Weekly wellness tips are a separate, optional email service for people who have attended a coaching session. If you opt in, you may continue receiving them without another appointment booked. Reply “unsubscribe” to stop. Opting out does not affect your coaching.
Do not use email or the discovery call for urgent medical help. Contact the appropriate local emergency or urgent-care service.
Cancelling or rearranging
For an individually paid appointment, cancel at least 24 hours before its start time for a full refund or rescheduling. Exactly 24 hours before is within this entitlement. Email a clear request with your name and appointment details.
With less than 24 hours’ notice, a refund or rearrangement is not guaranteed. We will consider exceptional circumstances and apply any charge fairly, taking account of costs saved and whether the appointment can be filled.
If we cancel, you may rearrange or receive a refund for the cancelled service. To rearrange a booked package session, contact us at least 24 hours beforehand; this does not itself cancel the whole package. Unused package credits remain available with no expiry. After any applicable statutory cancellation period, they are normally retained for future sessions rather than refunded, except where the law requires a refund, we cannot provide the service, or an exceptional circumstance means a refund is fair.
Statutory cancellation rights
For qualifying distance or off-premises service contracts, you normally have 14 days from the day after the contract is made to cancel without giving a reason. You may cancel by a clear statement to our email or postal address; you do not have to use a particular form.
If you expressly request coaching to start during that period and then cancel, a proportionate amount may be payable for services already supplied, where the legal conditions are met. The cancellation right ends on full performance only with the required prior consent and acknowledgement. One session does not complete a three-session package.
These rights are separate from the 24-hour appointment policy. Nothing in these terms removes statutory rights or remedies for services not provided with reasonable care and skill.
Privacy and complaints
Read our Privacy Notice before providing personal or health information. Coaching consent, health-data consent and optional weekly-email consent are separate choices.
Send complaints to seekchronicwellness@gmail.com. We aim to acknowledge them within five working days and provide a substantive response within twenty working days. If more time is needed, we will explain why and update you. These targets do not limit your legal rights or your right to complain to a regulator.
Your legal protections
We remain responsible for providing the agreed service with reasonable care and skill. Nothing excludes liability where exclusion is unlawful, including death or personal injury caused by negligence or fraud.
Changes to these terms will apply to future bookings after they have been made available to you. Existing bookings remain governed by the terms agreed for them, unless we agree a lawful change with you. Mandatory consumer protections that apply to you are not excluded.
